How a DHI hair transplant works

DHI, or Direct Hair Implantation, uses a Choi implanter pen to place each follicle. The follicle is loaded into the fine hollow tip, and the pen creates the site and releases the graft in a single movement, so no separate recipient incisions are made first.

This gives the surgeon close control over the angle, depth and direction of every graft, and it can shorten the time follicles spend outside the body. It is the same principle as FUE for harvesting, with a different way of implanting.

Before any of that placement begins, the plan has to be settled, and that starts with the cause and pattern of your hair loss rather than with a graft number. Diffuse thinning, which is more common in women, is investigated for an underlying medical cause first, because surgery is not always suitable and can be premature. Once surgery is judged appropriate, the number of grafts is worked out from the size of the area to be covered. Your surgeon measures the thinning zone in square centimetres and multiplies it by a target density, commonly around 25 grafts per square centimetre at the soft front edge of the hairline, rising to 40 to 45 in the central tuft just behind it and around 35 across the wider frontal area. That arithmetic gives what the area would take rather than what you can receive, because the donor area stays the same size while thinning can spread, so grafts are usually held back for later and the agreed plan is often smaller than the calculation alone suggests.

Where DHI is a strong choice

DHI is often chosen for the hairline and for adding density in a defined area, where fine control matters most. It frequently allows a partial shave, and in smaller sessions an unshaven approach can sometimes be considered.

Whether DHI or standard FUE suits you better depends on your pattern of loss, donor supply and goals. Neither is universally better, and the decision is made together at your consultation.

Recovery and the growth timeline

The procedure is done under local anaesthesia. Redness and small scabs are normal in the first week, and for the first few nights it helps to sleep on your back with your head slightly raised so nothing presses on the grafts.

Transplanted hairs usually shed within the first weeks, then new growth typically starts around months three to four. By around two months many people are past the shedding phase, and the fuller result is generally judged at about twelve months.
