Why afro hair needs a specialist approach
Afro-textured hair curves beneath the skin as well as above it. A standard straight punch can cut across the follicle and damage the graft, so extraction has to follow the curl of each root, using an adapted punch and angle.
Cutting across a follicle is called transection. In a study of 18 patients with tightly curled hair, sharp and dull rotary punches failed outright or over-transected in 8 of the 18, with individual rates above 80% and one near 90%. A curved non-rotary punch stayed under 5% in every case, and under 10% in the harder donor areas of patients who also had acne keloidalis nuchae, a scarring scalp condition. A later series using one commercial device reported mean rates of about 3 to 6%, nearer 6% for coily hair, with individual maximums up to 15%. Both studies were run by the surgeon who developed and patents those punches, and neither used an independent assessor, so treat the figures as a reason to ask any clinic you consider what it measures, rather than as a settled benchmark.
